About Us


Eaglebridge Primary Care Network (PCN) sits within Cheshire East Place and consists of two surgeries based in Crewe: Millcroft Medical Centre and Earnswood Medical Centre. Both practices, situated in Eaglebridge Health and Wellbeing Centre, provide care to over 46,000 patients across Crewe. Together both practices work locally with the community mental health teams, social care, pharmacy, hospital and voluntary services to care for local patients. 

Our PCN works to improve patient care, experience, outcomes, and access by developing skills of staff, the services we offer, and integrated working styles. We aim to do this by building and developing partnerships between members of the practices and other care providers to facilitate a joined-up approach to patient care. A focus on population health will additionally ensure current health care interventions are directed to the relevant people and allows our PCN to better tailor future health and care needs to the patients. Through investing in the right digital technology, we aim to ensure the PCN remains up to date with new advantages and opportunities which can further benefit our current way of working.
